ABSTRACT

@#【Objective】To establish a reliable and reproducible mouse model of cisplatin induced inner ear hair cells loss and to make possible for further study of mechanisms of cisplatin-induced ototoxicity in mice from the molecular and genetic aspects. 【Methods】Twenty healthy mice were evenly divided into two groups in random. The mice in group A received a single dose of furosemide(200 mg/kg,intraperitoneal)followed by a single dose of cisplatin(1.0 mg/kg,intraperitoneal)1 h later,and the mice in group B received a single dose of furosemide(200 mg/kg,intraperitoneal)followed by a single dose of cisplatin(0.5 mg/kg,intraperitoneal)1 h later. Auditory brainstem response(ABR)and distortion product otoacoustic emission (DPOAE) were tested in both groups 10 d post the cisplatin-furosemide treatment and bilateral cochlea were taken out and fixed,one side cochlea were used for the surface preparation of total cochlear basal membrane and stained to count the number of cochlear hair cells from apex to base,the other was embedded with EPON 812 gel and then cut into half thin slices. Six mice were used as control group. 【Results】In both groups,the average ABR threshold of mice at each frequency were elevated,while the average DPOAE values were decreased. The average ABR threshold in group A increased significantly and the amplitude of each frequency decreased a lot,and DPOAE in high frequencies vanished;In group B,there was a minor elevation of ABR threshold with a slightly lower amplitude in higher- frequencies(16,20,32 kHz)but no amplitude change in lower frequencies(4,8,12 kHz)both in ABR and DPOAE. The surface preparation of Cochlear results in group A showed that nearly all the outer hair cells in the base had been destroyed,and most were destroyed in the apex;In group B,only the majority of cochlear outer hair cells at the base missed. There was no decrease of the numbers of the inner hair cells in both groups. Semi-thin cross-sections results also showed that the stria vascularis and spiral ganglion cells were close to normal in both groups. 【Conclusion】Co- administration of single dose of 1.0 mg/kg cisplatin with a single dose of 200 mg/kg furosemide may induce significant loss of cochlear outer hair cells in mice.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the efficacy and advantages of laparoscopy-assisted total gastrectomy (LATG) with D2 dissection of lymph nodes versus conventional open D2 total gastrectomy (OTG) in advanced gastric cancer.</p><p><b>METHODS</b>One hundred and twenty-five patients with advanced gastric cancer in the middle or upper third of the stomach were operated on from July 2005 to March 2007. Of the patients, 59 cases received LATG and 66 OTG with D2 lymph nodes dissection. Clinical data were recorded and compared between the two groups.</p><p><b>RESULTS</b>No patient in the LATG group converted to conventional operation with laparotomy. No operation mortality and no severe morbidity occurred in LATG group. As compared with OTG group, in LATG group operation time was longer [(330 +/- 71) min vs. (261 +/- 54) min, P =0.005] in LATG group, but with similar number of lymph node retrieval (36 +/- 13 vs. 34 +/- 16, P =0.450), less operation blood loss [(175 +/- 101) ml vs. (359 +/- 210) ml, P =0.003], earlier recovery of bowel activity (P = 0.015), and a shorter duration of fever after operation (P = 0.024).</p><p><b>CONCLUSIONS</b>LATG with D2 lymph node dissection in advanced gastric cancer is safe and technically feasible with better operative access and visual field, less operation blood loss and earlier recovery.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To reconstruct a finite element model of human middle ear and measure characteristic dimensions of this model and calculate the mass properties of the ossicles.</p><p><b>METHODS</b>The proposed method starts with the histologic section preparation of human temporal bone. Through tracing outlines of the middle ear components on the sections in AutoCAD2005, a set of exterior contours of the components is obtained. The three-dimensional solid model of middle ear, including tympanic membrane, ossicular bones, middle ear suspensory ligaments/muscles, are reconstructed using these contours in Unigraphics (UG). To prepare for finite element analysis (FEA) of the middle ear, all surfaces of the solid model are translated into ADINA, a commercial FE model package. Based on these surfaces, FE meshes of the middle ear are created, and material properties and boundaries are set up. The characteristic dimensions of this model are measured and the mass properties of the ossicles are calculated to confirm the accuracy of the geometric model constructed following the proposed method.</p><p><b>RESULTS</b>The three-dimensional finite element model of the human middle ear that included tympanic membrane, ossicular bones and middle ear suspensory ligaments/muscles was reconstructed. The accuracy of this geometric model was confirmed with the outcome of the characteristic dimensions of this model and the mass properties of the ossicles.</p><p><b>CONCLUSIONS</b>The proposed method not only provides an effective, convenient, economic, accurate way to reconstruct the three dimensional finite element model of human middle, but also provides a detailed knowledge of middle ear geometry that is required for finite element analysis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To design a tool for evaluating the sound transmission function of ossicular prosthesis and explore the impacts of ossicular prosthesis bead area and the effects of the mass of ossicular prosthesis on the sound transmission function in mechanical middle ear model.</p><p><b>METHODS</b>Two latex membranes were used to represent the tympanic membrane and oval window membrane. The ossicular prosthesis was fitted between the artificial tympanic membrane and oval window membrane during the test. Pure tune signals were used to stimulate the vibration of tympanic membrane. The vibration of oval window membrane was recorded by a laser Doppler vibrometer. The ossicular transmission function was evaluated by comparing the vibration velocities of oval window membrane. Two groups of titanium ossicular prosthesis with different head area and mass respectively were fitted into a mechanical middle ear model to evaluate their sound transmission functions.</p><p><b>RESULTS</b>The feeling threshold curve of mechanical middle ear model (MMEM) was similar to the hearing threshold curve of normal person. The transmission function of the prosthesis with small head area was better than that of prostheses with large head area at frequencies 1500-4000 Hz. The small-massed prostheses functioned better at higher frequencies and the large-massed prostheses functioned better at lower frequencies. But small-massed prostheses functioned better as a whole.</p><p><b>CONCLUSIONS</b>The MMEM was an idea tool to evaluate the transmission functions of different ossicular prostheses. Both the head area and prosthesis mass had an influence on the transmission function of ossicular prosthesis. So while designing the ossicular prosthesis or performing ossiculoplasty, both the head area and prosthesis mass should he taken into consideration.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the surgical treatment of benign prostate hyperplasia (BPH) and its concomitant diseases at the same time.</p><p><b>METHODS</b>One hundred and fourteen operations were performed for BPH patients, including transurethral resection/vapor of the prostate (TURP/TUVP), inguinal herniorrhaphy, internal urethrotomy, transurethral resection of bladder tumor (TURBt) or vesical litholapaxy, and the data were reviewed.</p><p><b>RESULTS</b>The procedures were successful in all cases. A follow-up of 3 to 60 months found a good outcome of TURP. There was no recurrence in 30 cases of inguinal hernia and 39 cases of vesical calculus. Of the 25 cases of urethral stricture, 1 had an obvious hypotension during the operation and 4 needed urethral dilatation after operation. Six of the 20 cases of bladder tumor underwent a second TURBt due to the recurring tumor which was far from prostatic urethra.</p><p><b>CONCLUSION</b>Inguinal hernia, urethral stricture, bladder tumor or vesical calculus can be treated simultaneously during TURP.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the correlated factors and clinical features of fungal rhinosinusitis.</p><p><b>METHODS</b>The clinical data of 110 patients with fungal rhinosinusitis treated by surgery and another group of 110 patients with chronic rhinosinusitis who were sampled randomly between January 1999 and June 2004 in our hospital were retrospectively compared. The correlated factors and the clinical features of fungal rhinosinusitis were investigated by using the multiple factor Logistic regression analysis and chi-square test. The pathological types of 110 fungal rhinosinusitis were classified by using Gomori methenamine silver staining which was special for fungi.</p><p><b>RESULTS</b>The logistic regression predictive equation for fungal rhinosinusitis was : y = -8.713 + 0.496x1 + 4.575x2 + 1. 190x3 + 4.119x4 + 1.199x5 + 2. 698x6, P = exp (y)/[1 + exp(y)], in which the concomitant variables were course of the disease (x1), haem-nasal discharge (x2), headache (x3), calcified plaque in CT scan (x4), age (x5) and unilateral/bilateral sinus lesion (x6), respectively. The P value meant the probability of suffering fungal rhinosinusitis. Compared with chronic rhinosinusitis, the clinical features of fungal rhinosinusitis were female, over 40-year-old, course of disease < 3 years, headache, haem-nasal discharge, unilateral sinus lesion and calcified plaque in CT scan. Among the 110 patients with fungal rhinosinusitis, 34 cases were chronic invasive and 76 were non-invasive.</p><p><b>CONCLUSIONS</b>The clinical features of fungal rhinosinusitis are significant for the diagnosis, and it can be predicted by using the suitable logistic predictable equation.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To detect the expression of the glucocorticoid receptor-alpha/beta (GR-alpha/beta) mRNA in nasal polyp and normal nasal mucosa and investigate the significance of receptor-alpha/beta in GC-insensitive and GC-sensitive patients.</p><p><b>METHODS</b>The expression of GR-alpha/beta mRNA in 40 samples of nasal polyp and 30 normal nasal mucosa was examined by using Fluorescent quantitative PCR.</p><p><b>RESULTS</b>Forty cases were screened from the 80 cases after follow-up, including 26 cases of GC-sensitive and 14 cases of GC-insensitive. the levels of GR-beta mRNA in group of GC-insensitive [(5.72 +/- 0.58) x 10(2) copy/microg] were significantly higher than that of GC-sensitive [(4.82 +/- 0.28) x 10(2) copy/microg, t = -6.65, P < 0.01] and normal nasal mucosa [(4.44 +/- 0.35) x 10(2) copy/microg, t = -9.19, P < 0.01]. There was difference between group of GC-insensitive and other two groups of GC-sensitive and normal nasal mucosa (P < 0.01). The ratio of GR-alpha/GR-beta showed significant difference between the group of GC-sensitive (829.42 +/- 67.36) and that of GC-insensitive (535.70 +/- 89. 00, t = 11.74, P < 0.01).</p><p><b>CONCLUSIONS</b>The high expression of GR-beta mRNA and low expression of GR-alpha mRNA in nasal polyp show the status of "insensitive" in chronic sinusitis and nasal polyp. The GR-beta may play an important role in evaluating the effect of glucocorticoid in chronic sinusitis and nasal polyps.</p>
